Administrative Records of the Scottish Medical Service Emergency Committee. The forms were issued by the Scottish Medical Service Emergency Committee. A registration form was to be filled out by a practitioner not holding a commission and an intimation form by or on behalf of a practitioner on service. Both forms were supposed to be submitted before 31st December 1915. The intimation forms are not dated and a number of the registration forms were not submitted on time and date from 1916 (see Currie p53). The packets also included some correspondence where they related to the submission of a form or were attached to same. The forms were scored through with blue pencil which might mean that they had been processed and the information transferred elsewhere.
